Please CLICK HERE to fill out and submit this form for each participant. It would be helpful to receive this information as soon as possible. Please be aware that students cannot participate in our summer camp program without this information. 

Refunds:  If plans change or a program must be cancelled, you can switch programs (if space permits) or receive a full refund until June 1, 2021. Cancellations will be refunded at a 50% rate from June 2 to July 1. After July 1, no refunds will be available.
